Referrers Information

For GPs and Specialists who would like to discuss a referral:

• Dr Reyes can be contacted on his mobile through St George Hospital switchboard at (02) 9113 1111.
  
• All referrals will be seen within 1 month.
  
• Urgent referrals (highly labile or erratic blood glucose levels, thyrotoxicosis, hypercalcaemia, and pregnancy-related endocrine disorders) will be triaged accordingly after discussion.
  
• Dr Reyes can organise endocrine dynamic tests at the St. George Hospital, including short synacthen test, water deprivation test, insulin tolerance test, etc.
  
• There will be prompt correspondence and handover back of patients.

Transition and Young Adults Diabetes Service (TrYADS)

• Weekly Tuesday morning clinic
  
• Multidisciplinary clinic that bridges the care of 16-25 year olds with type 1 or type 2 diabetes
  
• Staffed with diabetes nurse educators, dietitian, and psychologist
  
• Liaison with the Paediatric Diabetes Service
  
• Available finger prick HbA1c measurement, insulin pump download, and pathology services

• Weekly Friday morning clinic
  
• Multidisciplinary clinic staffed with diabetes nurse educators, dietitian, and psychologist
  
• Liaison with the Antenatal Clinic and Obstetric Unit
